Rock Flower Anemone died over weekend

I had a rfa shipped in from TSA last week and all seemed to go well the first few days. It had walked around the tank and finally found a spot to settle but it wedged itself in an awkward position. It’s a work tank so I left it on a Friday and came in this morning and it was pretty much melted and smelled awful. For future rfa care, if it gets wedged in a tight slot like this do I need to move it? This time I thought if I just left my hands out of the tank it would be better. Anything I could have done wrong for this exact specimen? Nitrate 20, phosphate 0.08, alk 9. I’ve got the before and after photos attached.

Is the 2nd picture when you first received it ? It looks like it was ripped or cut

Yes. It was pressed against the glass so I assumed its mouth was just smooshed against it. I’ve also never had an rfa before so I could be wrong
