Fish only 400g tank and cannot catch to put in hospital tank. I would appreciate any tried and true recommendations for treatment. I've used Melafix for open wounds and fin rot but never had a fish with hole in the head.

A photo helps, but it seems to me a lateral line disease, this in aquariums normally caused by electricity leakage, it measures if there is any leakage, it would not remove it, it only produces more stress, greetings.

I'm also thinking leaking voltage from a heater or pump. Or, do you run alot of carbon?

Ammonia 0; Nitrite 0; Nitrate 80 that I've water flowed 100g today and 100gal tomorrow to bring it to 40.

I don't run carbon. How do you find leaking voltage?
Do you own a voltmeter/multimeter? Have you ever touched something metal while working in the tank and gotten shocked?

Head and Lateral Line Erosion (HLLE( common in tangs and other saltwater fish. As the name states, this disease affects the lateral line organ and skin covering the head and face of the fish. There is no specific cause but generally attributed to poor diet, use of carbon heavily, water conditions and even stress. My suspicion here is diet.
Feed at minimum:

Spiriulina brine shrimp
LRS herbivore diet
Nori seaweed basted with garlic extract
Mysis shrimp
small plankton
Formula 2 Frozen and flakes

Add garlic to food 2-3x per week and selcon vitamin to food 2X per week on other days

Stray voltage does NOT cause HLLE, it is a "red herring". Most of what you see is induced voltage from submerged pumps, the fish are not grounded, so there is no current flow.

Did you try to post a picture? I saw one post from you that was blank....

I have seen so many post in this forum about stray voltage, I don't think stray voltage will harm a fish. Electricity will travel the path of least resistance, we can measure it's presence with a voltmeter because the other probe is grounded. Fish in the tank will not feel it because they are not grounded. One other fact is that the conductivity of saltwater is higher than the fish so it doesn't need to go thru the fish.
