Seachem ammonia alert badge doesn't work

Starts yellow.
Put in my QT: turns green ("alert").
Fish die on day 5: still green.
Take out of tank, put in water: still green.
Put in new QT along with ammonium chloride: still green.

Which part of this makes you think it's not working? Can you drop it into a cup of fresh saltwater with a few mls of vinegar? You've gotta leave it for a few hours but the lower pH would reduce any amount of free ammonia present in the salt mix and get it to turn yellow, if it's working.
 
Ammonium chloride converts basically instantaneously into free ammonia when it's added to water (ph depending) idk what you're seeing that says it's incorrect. It takes up to 2 days to start working, and at minimum 4 hours to show a decreasing level of ammonia back to safe.
