It is unfortunately a flatworm, indeed.
There is a lot of information about them, how to identify the exact type you have as well as different eradication strategies available online. I highly recommend you to read over the different approaches and plan this out, you'll most likely end up doing weekly treatments for a couple of months to completely eradicate it.
