Small Hammer Died

Just removed a single head hammer from my tank. Guessing it died in the past day or so. Still looked like some of the flesh was still on the stalk. This is the first LPS that I have lost. Everything else looks fine. This tank has been great and all the LPS corals have thrived in it. I have read many of the horror stories of peeps losing everything really fast. So, my questions,

1. Anytime thing I should worry about?
2. If so, what do I need to look out for?
3. Preventatives measures that I can take?
4. Anytime else anyone can think of?

The reason I ask is I had read several times that brown jelly smells terrible. I had a torch doing poorly one time and while i hadn’t seen any signs of jelly i took it out of the tank to inspect and the smell was pretty terrible. I got it out quick.
