I recently added purigen in the tank . Dont know of this is the cause , but this hammer been like this for the past 3 days before this everything was perfect.

Zero nitrates is bothersome, how about flow and light?

Low to moderate flow and lighting is at 15k 40% intensity, its was perfect this just started happening a few days ago. Im skipping a few water changes to bring up nitrates and see if that helps. Im starting to think its my pair of clownfish who are next to it and they are stressing it.

I definitely think nitrates can play a role. Are phosphates actually.5 or .05? If there .5 that’s another issue in itself. I would feed a lot to bring up nitrates. I don’t believe clownfish will have that much of an impact on it. If their hosting in it most of the time their okay, sometimes hammers won’t like it but I’d look at nutrients first

I would lean towards no nitrates as well. Unless you're spot feeding this guy often he's at least starving a little.

Clowns could be bothering it, but I would think you'd see them nipping at it if that were the case. I feel like this is less likely.

I would also check around the base For euphyllia eating flatworms and their eggs.
