Torch head popped off.

Hello fellow enthusiasts. I’ll keep this short and sweet. My wife has a few torch’s in her tank and no other corals besides some zoas on a frag. For weeks her new “Halloween torch” was big and fluffy and waving in the wind. The next day it was sucked in a looked mad. The VERY next day POOF his head popped off. I don’t understand what happened sooooo fast. Her other torch coral is sucked in today and looks upset, I’m worried it is going to happen again. Parameters all seem to be in check. I know for a fact no other corals are bothering it. She has a purple tang, sail fin, clownfish, snails, emerald crabs, and one shrimp (the dark red one) I forget the name of those. Peppermint shrimp maybe?? Can one of them be bothering them? What else should I check for? Please help before my wife goes crazy lol.

Side note. She has had this torch for around 4-5 months total. I said “new” but it’s not brand new.

Tank size and list actual parameters.
Full tank shot in white light, photos of corals in white light.
Do you mix your own salt? Do you use an RO system?
Need some tank history to diagnose :)
