Unhealthy bubble tip anemone?

I’ve had this anemone for about 3 months, it has gotten caught on the nem guard of my mp40 twice. First time it recovered almost immediately, second time was about 3 weeks ago and it’s been hiding in the rockwork since. Parameters are fine, corals fish and shrimp/snails all fine. I put the nem in the acclimation box today because it was basically hanging off the bottom of a rock in the back of the tank completely deflated. Any tips to help it recover?
