What’s happening to my torch coral?

Can't see much in this picture. Looks like something might be eating it.

Looks like polyp bailout on the one head. What are your tank parameters like? What’s your flow? High phosphates (really high) led to polyp bailout for some of my torches a while back. Critters bothering them can also do it.
