What causes polyp bailout?

I have one acan, a frogspawn and a favia that are all experiencing polyp bailout. I can't figure out what's going on. My other corals are doing fine. I haven't moved them. Parameters are phosphate 0ppm
Nitrite 0ppm
Nitrates 10ppm
Calcium 400
Mag 1380
Salinity 1.026
KH 8.0
Temp 78°.
I keep finding polyps from the affected acan lying on the sand. What gives?

i experienced polyp bail out when my nitrates were above 100ppm but I hadnt realized it because being a newbie a few years ago i hadnt done the test correctly so my nitrate kept reading as 5ppm.
I would say polyp bailout is usually caused when there is a drastic change in a parameter, either salinity, temp, nitrate in my case.

Following. Had a frogspawn recently have all of the polyps bailout out within a week. Can they get bacterial infections? If so, I think that my was cause. Or due to it being stressed when I moved corals around searching for a worm

I've been wondering if it could be a bacterial infection or some sort of disease myself.

There's no polyps left now it's just a skeleton.

Oh no :( People here told me that even experienced aquarists have issues with Acans while beginners don't. Acans just sometimes don't like the tank. I have similar water parameters as you and my acan is slowly shrinking
