What fish is the suspicious fish?

My tank is doing amazing, corals wise, however every now and then, i find one dying head on my hammer especially. I have a foxface. Hippo, yellow tang, tomini, desjardini, wrasses, royal gamma, is it the foxface? I also have a big hippo
 
Do you have any peppermint shrimp? that is what recked my hammer

You sure they are being eaten and not just dying?
 
If you ordered live rock from Florida, I swear those have limpets that love lps. I woke up once to a nearly dead blasto skeleton (was a healthy colony two days prior and just started shrinking) and it had two limpets on it. I picked them off and would pull them out anytime I saw them and eventually it recovered
