Nothing. Mine do this from time to time as well. Unless you see pieces of it floating off or after an hour it stays that way nothing to worry about. They are almost like bta when they do this as well.
Mine do this especially after feeding the fish as if it has gotten a little snack too. Do you have angels or butterflyfish in that tank? They can bother the hammers.
I noticed you moved it from your rocks to your sand
Try not to move it too much especially if it's in a weakened state.
I would rx dip it
Then place it back in in a low flow medium light spot
I had a frogs pawn do this same thing after a bad water change it looked like it was going downhill fast but it's made a huge recovery.
Check your water params keep up with water changes and do the rx dip.
