Why is this happening? Stream 3

This is the second time in less than a week I come home to see this. Rectifier blown out, the impeller half way out.

The second Stream 3 on the other side works just fine.

Any thoughts?

most likely the plastic nut/screw in the back needs to be adjusted adjusted. Take it apart and the back area there's a white screw it outward away from the impeller side, slightly.

Is the propeller being fully seated when you assemble it? Has the drive unit or bearing ever been replaced?

Thank you both for the assistance. However, upon further inspection, it was a snail that caused the issues. Bit of a broken shell and a cut on his foot, no damage to the pump, I think everybody walked away lucky in this case.
