About 6 months ago I noticed a white spot on my yellow watchman and assumed it was from a scuffle with my one spot fox face who loyders by the watchman's cave. All seemed fine, but recently the watchmen didn't come out of this home for about a week and just reappeared today. The bottom half of his body is pale and swollen and seems like it may be paralyzed. I can also see him breathing heavy. Any idea what this could be?

He's very weak and gets blown around a bit by the wave makers.

There haven't been any changes or additions to the tank in about 6 months. And all other inhabitants have been acting normal.

Fish is lethargic and shows signs of injury either from aggression or mechanical injury. Are there other fish in tank and do they look?
Is fish breathing normal or labored?

Well, he has passed...

It looks like it had a systemic internal bacterial infection. If so, that would have been untreatable. One possibility is that the swelling was from unreleased eggs. These in turn can become septic.
