Anemone almost dead

I woke up today and found my anemone on the other side of the aquarium; middle right. The foot was damaged on the edge and I don’t have a photo of it. I posted another thread yesterday that looked like this. Today. It was on the other side. Is there anything I can do? I’m at school right now.

Phosphates are at 50 ppm and nitrates are quite high. Alk is 10.2. Ammonia is 0.01

There is absolutely no way phosphate is at 50ppm. Also what test kits are you using.
