Are there any changes to the flow? How is the rest of your tank doing?

There are different types of bubble coral. There are a few different shapes of bubble. Yours looks like a Physogyra lichtensteini or a Plerogyra discus. The OP's is likely a Plerogyra sinuosa.

Wish I could help. Mine thrives in NO3 15-20 and PO4 0.2-0.3 so close to yours. Your calcium is high but I'm not sure if that would cause issues. I've had my bubble over 7 years and it's thriving. That said I added a green one a few years ago. I placed it in the same light and flow as my other one and it only lasted a month. Sad part is I have no idea why one thrives and another dies.

It’s been almost two decades since I had one and loved but due to those long sweepers and the size of my my current tank it wouldn’t work.
