Apex variable speed problem

I got my apex about a week ago. had my light hooked up and working sence day one with dimming on the v1/v2 outlets. Well tonight i noticed that my light was off. so i got on fusion and seen the variable speed channels were all set at 255%!?!?!?! I hurried up and unplugged the apex and plugged it back in. It now says the right % but the v1 and v3 controls are on one outlet and v2 and v4 on the other. what causes this and how do i fix it back?
