Suggestions for places to get a tailspot blenny that is known for selling healthy fish
I second looking at your LFS and doing your own quarantine if you're concerned about disease. I don't know that they're available as captive bred at this point. TSBs are very hardy fish. As long as the fish in your LFS's tank looks healthy, you should be pretty good.

I’ve ordered from them a few times and they’re always healthy, small but very healthy.
Very small they definitely don't continue to raise them any longer then they have too. My LFS gets fish in from them periodically the yellow tangs are the smallest ive ever seen and the dragonets are only about an inch or a little under.

I’m restocking after losing all but one fish to velvet disease, I want to be extra careful this time around
Other then the fish being very small any downside to buying aquacultured fish online and is being small an issue as far as possibly being killed by an invert or other fish ?
Do brittlestars or shrimp kill tiny fish ?

Special care around food size sometimes will apply to this. My LFS advise that if your getting one of these little guys to at least throw them into a quarantine tank until they are taking food consistently.
