Blue tang laying on side

Came home today didn’t see the tang swimming around. Checked and he was in the rocks just kinda laying there breathing pretty hard. Fed some food with garlic and he ate but went right back to laying down

Note the mark on head and also about halfway down his body. Almost looks like injury or is this HLLE I know the clown gets territorial at night. Wondering what you guys think it is and anything I can do. Appreciate it

Edit also want to note I know it’s very malnourished It’s very tiny and I haven’t had it long at all. Wonder if that has anything to do with it ?

How long have you had it? Did you do any qt when you first got it? Any new additons/changes to the tank?

It’s not HLLE. It looked like an injury to me, but any idea as what could have caused it?
