So about 2 weeks ago my liutenant tang looked like it had ich but then disappeared. Now it has this brown spots on it and looks like the ich is back. I don't have a qt tank is there any medication that is reef safe.

you will need to setup a QT tank and treat all the fish with copper for 30 days and leave your display tank empty for 76 days to fully get rid of ich. on the second picture i can see the yellow tang and it looks to have ich as well in the photo

Fish are still swimming and eating. Is there any medication I can use without having a qt tank?

That is a very nasty bacterial infection. Here's options for you to consider. But I'd get that QT setup and dose both copper and antibiotics.

Just to let y'all know both of my tangs died this morning before I got the chance to setup a qt tank. Maroon clownfish, melanurus wrasse and snowflake eel seem to be doing fine. Don't see any spots on any fish. Don't know what to do next. Should I medicate or start qt tank. Any advice would help.

The parasites will still be in the tank. Clownfish are often the "last man standing" and eels because of their thick mucous coating can withstand the parasites better than most regular fish. But can be carriers.

The clownfish and wrasse can be treated with copper.

But the eel is recommended to be treated with CP or TTM.

Here's a chart showing the best treatments for types of fish:
