Dead Eel... what's next?

How old is this tank?
I think you should iron out this tank first, before you add anything else.
I would reconsider your stocking and do a lot of research before you add things. I think you would have better success adding fish that are smaller, like fish for 10-30 gallon tanks, this will let you keep a lot more fish and you won't have to worry about aggression.

Tank is established 2-3 years. 2 different cycles. When Iived in Florida, it was all nice fish. When I moved to Texas, kept same sand, about 20% or more of the water.
I tagged you because I would like to hear your opinion and reccomendations since you are my friend! Keep in mind I plan to upgrade the Foxface to a bigger tank in about a year. What smaller fish do you recommend? My father is wanting a Ocelaris Clownfish, but we already have then Maroon. I'm looking for Gray Head Wrasse.

I'm not a very big fan of clownfish, but I'd much rather have a ocellaris than a maroon. A grey head wrasse is to big for that tank imo.
 
I would trade the maroon in and get a pair of occys. That would be a step in the right direction.

HOWEVER, the blue velvet damsel may kill them or stress them out enough they die. So I would trade that one in too. If you want a damsel, get a springers damsel.

If you don't pick, mix and feed fish appropriately or have them in appropriate size tanks, you are going to have a bad time. Most fish are long lived if properly cared for.

Not sure where you read that but not true. Lionfish also need a bigger tank than a 45g. I suggest focusing on getting a bigger tank. Foxface will outgrow that tank in a month or so, they grow super fast. Also slow down, all the fish youre asking about should not be in a 45g. Get at the very least a 125 six foot for those fish youre talking about.
