Looks like your pic or video didn't post. Could you please post some clear pics using your whitest lights, turning off all your blues for clarity? A short YouTube video can be helpful too.
 
Strong odds favor ich and not velvet if the fish has spots and is still alive.

Agree that a white light photo or a video of the fish would make diagnostic sense.
