Dying copperband butterfly fish

I woke up this morning seeing an area of what looks to be missing scales and some blood on my copperband. I immediately placed him in a quarantine tank as well as doing a freshwater dip. Not sure if this is an injury or some sort of infection/ parasite. No rock inside of the tank is able to be moved. Any ideas ?

How long have you had him? Any new additions to the display tank recently? Is he breathing fast? Showing a feeding response in quarantine?
I’ve had him for about 3 months. I haven’t added anything with the last month. He was breathing very fast, with barely any feeding response. He passed this morning, still unsure of the cause.

it always sucks to lose a fish, especially one that seems to look into your soul. I see some frayed fins, looks like a possible injury? Maybe chased and hit by the blades of a power head?
