Dying toadstool??

Hi, a little while ago my toadstool went from nice and healthy sith great extension to this..

Whats going on!? Its been weeks now?
 
They can sulk for a while, but make sure it has enough flow, it likes higher flow. The gravel also looks very white and clean in the picture, ensure there are measurable nitrate and phosphate in the water.

Sharing more details about your tank and water parameters will help get more suggestions.
 
Any recent changes in your tank? What about parameters?
 
These are taking at 10:40pm so alk little low side

Alk: 8.3
Ph: 8 ish
Ammonia: <0.02ppm
Nitrate: 40ppm
Nitrite: 0ppm
Phosphate: 0.5ppm
 
Any updates on how it's doing? Toadstools can be moody but often get their attitude in check after a day or two.
