Skeleton is 9"x15" with 8 heads and now 24 mouthes- it had 23 just a few days ago!

Judging by your rock your tank is fairly new and it’s pretty hard to keep those in new tanks. Also you should place it in very low light and not on top of the rockwork.

The tank is well over 3 years old- that’s just a new piece of rock I picked up
 
I have also been keeping LPS for a while, I prefer to keep it high up with medium flow and medium light
