Maybe something worked its way into the tube? If it's not too late, then try opening the tube to see what else may be inside, like a predator of some kind. If it only came halfway out, there could be body damage on the back half.

It came out post mortem. I didn't see any damage on the back half, but something could have been bothering it back there.

I've had two come out of tubes. One came halfway out and just kind of chilled for a day or two, after which someone took a chunk out of its side. Kind of all downhill from there.

The other, which looks similar, but is just a bit smaller, was spotted out of its tube in a Florida liverock a few nights after receiving the shipment. Needed a bigger hole to hide in maybe, 'cause he's doing just fine - an inch and a half to the right.

Stress can drive them out, but since it kept its crown I imagine it was more of a location problem. Mine came out of its tube when my goby moved in next to it and kept burying him. It tried shuffling around, but eventually was buried to death by my pistol shrimp
