Frogspawn 's head spawn coming off!!

I have one frog spawn but these day i found some epoptic like frog spawn's head flying around the tank,so today i trying use the coral feeding tools to blow the frogspwan, and a lots head spawn shape easy come off, so is something wrong with the coral? Thanks everyone!

And just check water po4 under 0.25ppm no3 under 5ppm

Too much flow can rip the polyp off the skeleton.
