Hammer coral dying

Hello, I have a hammer coral that I got a few weeks ago. the first 2 or 3 weeks it was doing fine. but it has slowly started receding, and is now not looking good. any idea what might be off? Tank is about to be 4 months old, other corals look fine.
Alk 8.7
Calcium 410
Magnesium 1320
Nitrate 4.9
Phosphate 0.06

PH is about 8, and last I checked Par it was about 50
looks to be to far gone, but you never know. I would up the lighting to 75, but it may be to late for this guy. not saying lighting is the problem. There can be all kinds things to cause this, other corals, contamination of the water, pests, fish, etc
