Hammer Coral ???

I woke up this morning and my Hammer coral had detached itself from its skeleton and found it somewhere else in the tank. Has anyone ever seen this and if so what do I do? I read a post that someone put it inside some shelf to keep it alive.
 
Typically once polyp bailout has occurred then survival of the coral is rare. There is a thread where a member actually kept the bailout alive so it can be done. Not sure on the thread though.
