Help. Lobo developing hole in the side

Hi I just noticed my lobo is getting a hole in the side of it. Under the colored tissue. Is it splitting or should I be worried?

Any water chemistry issues lately? Could something have fallen on it or tried to eat it?
 
Nope. I test frequently. And none of the fish I have currently pick at coral. I have a few snails. And three peppermint shrimp. Confusing
