I jusy got this angelfish Sunday 8-7-16 , it looked fine at my LFS I've gotten all my fish from there. I've given it a dose of garlic extract. Don't really want to use harsh treatments. Any ideas?

It means your entire system has ich now, as harsh and difficult this may sound. It's highly recommended to remove all the fish out of your system and treat them on a separate QT tank and let your tank go fallow (fishless) for 78 days
Copper treatment is the most common method of treatment, although there are other alternatives such as Hyposalinity and TTM.
You can learn a lot more about it on "Fish disease" forums. @Humblefish has all the information one would ever need.
Best of luck

Other remedies are snake oil.

The other option is managing ich, but these days that's a serious gamble that won't end well it's just a matter of time before velvet or a strong stain of ich take over, and you will start at square one with most of your fish gone. This is what makes people leave this hobby en masse. The way I see it, do it right the first time and follow the rules and the long run benefit pays dividends

Garlic won't do a thing for you here. Treatment is needed and if it's velvet, then it's needed asap. Sorry hun.

Take all the fish out and either do a hypo salinity or copper treatment (if it is ick) hard to tell with the pic. Google saltwater ick and it'll give you lots of information. I recently just did hypo with great results
