What could be the issue and is this an easy fix?
 
Does it just skip back and forth? Have you tried cleaning the chamber? Sometimes giving it a little spin by hand will get it going on other pumps.

Citric Acid bath could be beneficial.
 
Yes.. I did clean and reassemble but issue persists

Does it do the same thing with the cover on? It looks like the shaft should go into a front bushing to prevent it from cocking.
Yes the issue is the same even with the whole thing assembled
