I need help with my Red Tail Trigger.

I have a red tail trigger that has been in my tank for about a week now. When first introduced she settled in nicely. Swam about the tank out in the open following me, coming up to the front display. Eating good as well.

We did recently add a purple, yellow, and gem tang this Friday. She seemed fine with the new addition, swimming about yesterday which is Saturday the 17th. Today Sunday she has displayed stress behaviors, she is now hiding in a new cave, breathing heavy, opening and closing mouth rapidly and on occasion barrel rolling in the cave. She does on occasion come out and swim about the tank but retreats back to the cave.

What could be the issue? She has no signs of disease, no sunken belly. All my other fish are fine, also have a blue throat the same size swimming fine. Someone please make a recommendation or offer an opinion.

In the tank there are two bengal cardinals, one flame angel, a Midas blend, naoko fairy wrasse, a blue throat fairy wrasse, two diamond gobbies, two clown fish, a royal gramma, one fire gobby, two small blue regal tangs, one yellow tang, one purple tang, one gem tang, one blue throat trigger, one white tail cole tang, and the red tail trigger.

0 ammonia
0 nitrite
14.7 nitrate
1.024 gravity
78 degree.

Tank is 200 gallons. All fish are less than 5 inches.

Did you quarantine properly. It maybe not have shown any signs of diseases until now especially if nothing was harassing it. I would recommend to set up a qt tank to monitor it until you can figure out the issues just in case

Testing with Hanna Checker. Except for Ammonia. Using API for that.

It seems labored: rapid gill movement and mouth movement. Eyes are clear, not hazy or cloudy. Only fish in the tank doing this behavior. Every other fish seem to be doing great.

The other fish swim in and out of its cave but doesn’t seem to be bothering it. She is the biggest fish in the tank.

These fish are prone to flukes. I suspect flukes and treatment can be done in tank although I favor separate tank using PraziPro dosed at 85% of recommended and apply initial dosage known as an interval for 8 days, do a water change and do one more 8 day dose interval
Use airstone with prazi as it does reduce both oxygen and appetite. If running a skimmer, leave cup off first 24 hrs as skimmer will go nuts from the glycol solution in Prazi

I agree, it could be gill flukes. With the newest fish showing the symptoms, that makes sense. Here is a link to treating with PZQ:
