Doesn't look healthy. I'm not an expert but poor water quality or poor lighting can cause this. Maybe another expert will chime in!

You're going to need to give up the goods for help on this one:

Parameters
Lighting Schedule
Age of Tank
How long have you had it
Anything you think might be more relevant then "what's wrong with it?", as the picture tells us not much at all other then there's a problem
 
True ima give u guys the specs in a few I have water flow it was fine for 2-3 weeks I'm running on 75 gallon tank the people at Petland told me to feed it MICROVERT with a syringe directly

I don't know how petal d is by you but from my experience unless you get it out of the box don't do it and do t trust there knowledge they will sell you anything possible . So there the type of people who put a box filter in a reef tank lol
 
Get some water flow and make sure the pieces of food you feed him are small or it will hurt his mouth.
