I saw this tang at a LFS. Ask the attendant if he know which species it is and he said it is a Surgeon Tang and even show to prove that it is a "Surgeon Tang".
Anyway, he is tiny, only about 1.5 inch, has white tail and have dark scalpel. I am sure he will change a lot once he get large, and likely to be a large tang. Picture #1 is most accurate in term of shape of the fish but picture number 2 and 3 are clearer. The shape of picture #3 is miss-leading due to the angle of the image.
Thanks.

I have asked if the supplier knows the origin in the past, sometimes that helps. A quick online search lists these 2 as possible identities but im no expert...
Acanthurus bahianus / Acanthurus chirurgus

Well, whatever species is this tang, he will stay at the LFS. I am a sucker for tiny fish but I really don’t need another big tang in my tank. He is so tiny and cute but will likely be a dabbed color huge 10+ inches tang in a few years.
As tempting as it maybe. I am not going to get him.

I believe it to be acanthurus blochii, some aren't as patterned as others but I feel like I can see some of the patterning that some juveniles display.

I’d venture ringtail also. So hard to tell with juveniles though. Couple of months ago I bought a juvenile vlamingi and unicornis .... looking more and more like two vlamingi. Oh well

Thanks @ca1ore and @4FordFamily
Thanks for the ID. It would get way too big for my tank. I make this mistake many years ago. Exactly the same situation. I had a 180 and bought a small unusual tang they I cannot completely ID and have to rehome him in record time. I was lucky to have a friend who have a 500 gal tank at the time.

From the pic I believed that's the same tang I have. Its a lined bristletooth or Striated surgeonfish. Mine was like that. Only thing his tail has lost all his white and turned to a dirty yellow. Depending on his mood I can see stripes along his body and dots on his face.
