Not dead but certainly not happy. You're going to have to provide more than just a picture though. We need info on tank, water parameters, etc.

how long have you had this hammer?

did that bottom left head under the 2 heads just die?

its not dead thats for sure, but you can see where the flesh has receded. the bright white skeleton by each head.

water parameters etc will help
