Last few visits I noticed these white dots on my grandfathers tang. We don´t know for sure and the dots are not visible under the white lights.
The tang is still eating well and swimming around normally. Sorry for the bad picture I can take some new ones on the next visit.

Welcome to Reef2Reef!

Tough to see under blue lights. You could try posting a video under white light, the spots might show up better that way.

Given what I see in the photo though, it does look like a moderate case of ich (looking at its left pectoral fin).

Let me know if you need treatment advice.

Thanks for the quick response.
I will ask my grandad to send a video under the white lights.
We do have an extra tank that we can setup for QT. We would really like to hear youre advice.
Thanks!

Two options would be hyposalinity or coppersafe. Both will take about 30 to 40 days to treat.

Hyposalinity thread is here:

The coppersafe treatment is part of our quarantine protocol:

Yes, that does seem to be marine ich. The big question is - why don't any other fish have spots? It is pretty common for one fish to develop a few spots before you see it on any other fish, but this tang has a lot of spots, and that means it has had ich for some time, and i can't tell you why it hasn't spread to the other fish yet.

Yes thats really weird especially since we put the tangs in the tank at the same time.
Thanks for your help!
