I bought this guy a few weeks ago. And he has already split. But the last couple days this one head looks like this ?

Looks stressed to me. Always trouble shoot water chemistry first. Looks like you have some algae growth going on so I would start with water quality.

Water looked all good calcium is a bit low but not crazy. Algae on the back looks like it's going away. Tank is about 5 months new.

I have had a slight algae growth right dead center of the tank on my rock. And holy heck do I have huge brittle worms. They actually come out to feed when i feed everything else.

Bristle worms are not normally bad. They eat what waste is left in tank. Water chemistry is key. Some corals will take time to adjust it depends on water conditions. Some will not.
