Coral beauty has 2 white spots by its fins .. is this ich and what do I do

No comment on potential Ich treatment but could it be sand stuck to him? I can't quite make out what substrate you have in the background.

May be sand opposed to ich but before a proper assessment can be made, please provide pics under white lighting or have someone aim flashlight at body while you take pics. Right now would be guess work

I have pretty fine sand. So it is possible. he swims in and out of the rocks all day. Sand can stick to the slime coat like that?

Okay I will try to get a better picture but I feel like he knows I’m trying to see that side and it’s messing with me !

Is it possible it could be lymphocystis? How fast will those 2 spots spread of it is Ich? Is it okay to just monitor and do nothing but a water change?
Lympho generally has large dots and starts at pectoral, caudal and upper doral fins

With these pictures one cannot make even a guess. Do the particles move? the history of the fish, your tank, etc. ? there is IMHo - no 'official' zone where lymphocystis 'begins'. - Usually its on the fins - yes - but - which ones - IDK. It looks like Lymphocystis to me

Looks like it could just be sand. Monitor for now and see if the spots multiply. Ich spots fall off and come back every 3 days or so.

Looks too large to be ich (unless the angel is really tiny). I suspect bent/broken fin rays or Lymphocystis.

Is there anything I can do if either of those is the case ? I just did a 15% water change and will be keeping a close eye on it

No - damaged fn rays will heal, but it can take months. Lymphocystis is a virus that almost always goes away by itself, no treatment needed. This may get worse before it gets better though.
