Large Black Spots on Puffer

I just noticed my puffer scratching the other day. Upon close examination, I've noticed a good number of black spots on the top of his back/head. They are too large to be black ich. If anyone has any suggestions, I would be very greatful. I'm wondering if it's Paravortex, Piscinquilinus. I have only a few fish in the tank. No, new members have been added to the tank in over a year. The tank has been disease free for a very very long time. The other fish are fine with no sign of disease.

Thanks for taking the time to reply. I really appreciate it. To clarify, are you referring to the smaller, grayer/black dots all over? The larger, jet black spots on him (maybe 10 or so) are part of his coloration and have always been there over the years. I've never seen these acne looking spots before and I've had him for many years.
