I understand this is an East Indian Ocean morph of Exquisite Fairy Wrasse and have been watching for one to become available. Are they ever sold as such? Any idea who might carry them? Maybe sellers that get shipments out of Mauritius?

Wish I could help you there, but I haven't seen any of those collected in years. There's only two exporters on Mauritius, and I know one of them doesn't collect them - can't speak for the other.
There's a small chance a Madagascar collector could offer them in the future (just speculation - I don't have any lines of communication there).

Im guessing on a lubbocky or a flavidorsalis.

It's nearly impossible to tell females apart in the complex. Unless you know what region it was collected, it's anyone's guess right now.

Last question... How much time does it tales for a female to transition into a male?
That would require a crystal ball... :) A few days to years. And a slight chance of never.
It's dynamic based on the hierarchy of the tankmates.

Thalassoma lunare.
Not suitable for a reef tank IMO. Others may disagree, but the temperament is not kind. All motile inverts are tasty. Coral safe, though.
