I tried to do a photo search and failed. Tried to look it up the old-fashioned way (with words!) as well. Failed again. But hey! It's the thought that counts!
 
Measure it, draw it, get someone like SendCutSend to make another. You'll have to tap the hole though for threads.

Does it have to be just like that?

TNutz has plates for mounting casters and leveling feet to 8020. They are bigger tho
