Checked on my fish in copper QT for a ICK outbreak, they have been in for 7 days found my 3 year old Foxface dead. He had no sign of ick when I QTd him (treating all fish) 4 other fish are doing fine. I've attached a picture if any has a idea of what killed him.

Sorry to see and I agree there are no signs noticeable. Its often near impossible to assess a fish once dead but Ive seen impact with glass or object but the mouth and nose area seems unaffected.
Are you using cupramine or coppersafe and at what dosage?

Are you using the Hanna HR to measure the copper?

What size is the QT?
Are you testing it for ammonia?

I agree that this may have been a failure to navigate a smaller tank after being in a larger one for some time - long term captives often don’t move well.
