Melanarus Wrasse suddenly lethargic

My male melanarus wrasse is suddenly lethargic. The day before he was active and eating and suddenly today he is not eating and lethargic. He is just laying on the rock work and occasionally moves. I have never seen him just laying around he is a very active swimmer. He normally has a good appetite never willing to turn down a snack. I have had this wrasse for almost a year. I mentioned he is male because I also have a female melanarus in the same tank. While the male chased the female at first they now seem to have bonded. No aggression toward him in the tank. Every other fish and invertebrate is completely fine. Please help!! What could be going on with my wrasse?

Need clearer pics of fish and some history. . . .
What other fish are in tank with them?
Any change in tank temperature-ph-salinity or recent water change?

Any notable damage to mouth and is fish breathing normal or labored?
 

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
