To me looks like velvet.

Interesting, when I had Velvet few years ago, the fish died very quickly with no sign

Thus appearance looks to be flukes which this fish is susceptible to and you will need to quarantine fish and treat with praziPro at 85% of recommended dosage for 7 days (one application) , change water and apply another dosage lasting 7 days

That's not velvet. It is either very late stage ich, (most likely!) late stage Neobenedenia flukes, or both.

It is going to be very difficult to save this fish, as any treatment will take time and this fish is on its "last legs" so to speak.

If it were my fish, I would attempt to treat it with hyposalinity at a specific gravity of 1.009 for 30 days. That will resolve ich, as well as Neobenedenia.

Damage control consists of knowing what other fish this one has been exposed to and do they show symptoms?
