Need help with hammer

Had this hammer for about 4-5 months and two of the three heads look like skeletons. I don’t know if it’s dying or splitting, would love some advice.

It looks like the polyps have either been eaten or they have bailed out.
If you can share more details on your tank - lighting, parameters, flow, livestock, etc... - then folks might be able to offer a better guess.
 
When they spit you won’t see skeleton. A new head beaches out from the flesh of one of the existing heads. I’m sorry to say the exposed skeleton is a sign of stress. Maybe chemistry isn’t good or too much light. The remaining head doesn’t look good either. I think it should be extending the polyp more. Hope all turns out well when you test your water.
