Why must Nitrates remain under 20 ppm?
Ammonia starts to climb and saturated oxygen drops and fish become stressed

Serious question, shouldn't the medications work regardless of the level of nitrate? After all you have to expect some level of stress from the fish regardless once you start medicating (generally).
The medication itself should warn you if levels of nitrate shoud not surpass 20ppm shouldn't it ?

Meds will work yes but meds also reduce oxygen to a degree and affects bacteria in which ammonia and nitrate CAN climb. Its more of a caution as for use of many meds.
