Orange mini Hammer not happy?

I have had an orange mini hammer frag since December. It had been looking and doing great until about 2 weeks ago, even grew a 2nd head. Lately it has not been fully opening and not sure why. Only things I can think of it my no3 is low but it always is and none of my other euphilia are unhappy or it might be splitting at the 2nd head but don't know if that would cause it to not open fully. Nothing is picking at it and no pests. Tank is a 75g, 3 years old and super stable, mostly acros that all grow well and good color. I am feeding heavy along with Acro Power, Reef Riods, phyto and Noe Nitro to supply no3. Any info would be great.

These are very challenging at times. Some things that make them unhappy are:
Too much water flow
Too much light
Pests such as nudibranch, flatworms
Calcium below 380
High phosphate or nitrate
