Also check the dates on your test kits. They should have an expiration date somewhere. You said you use salifert, Red Sea, and API?

I use API for the ammonia, nitrates n nitrites. And I have both Salifert and Red Sea to test the others. I'll use Salifert today to test since I've been testing for the past two months with Red Sea.

So........ the anemone is bleached. there are no healthy bubble tips that are white. did you buy it this color? low no3 won't necessarily bleach an anemones ,but high no3 and/or po4 could cause it to become unhealthy in several ways. stop trying to feed it, don't touch it, just get your po4 down. it's not dead......yet and it can be saved.

Is that the fluval marine and reef 2.0? That's what I'm running on my tank and my rainbow bubble tip is very happy. I have w standard 29 and he rest on the bottom of the tank near the sand and seems to get plenty of light. Was the nem white when you bought it? If so then he was bleached from the get go and already in poor shape.
Yes it is a fluval marine 2.0

I'd get your mag up to 1350

Alk imo is a bit to high

I still don't think those lights are good enough for a reef tank especially a 90g

Unless you have everything 6"-8" from the light I'd say it's not enough for nems or sps in a 90g

I have an 8 bulb coralvue t5/ 3w per royal blue led built in then I added 4 24" full spectrum led bars each led is 3w
