Small clown low to sand

Smaller clown in my pair of clowns was very low to sand seemed and seemed to be struggling to swim, went to feed them again and he instantly swam up to surface and seemed to be doing better, maybe flow is too high and he cant handle it? But flow has been the same for over a year. Help!

But if you say it swims to the top for to get relief, it might be low oxygen. You should have a powerhead aimed at the surface, the surface should ripple to help oxygenate the water.
